Send a Verification
The agency will send verification of your licensure status to another state or organization on request. The fee to verify a license is $50.00. The Board will mail verifications to multiple addresses if requested at the time the fee is paid.
If you are requesting that verifications be sent to more than one address at this time, include a separate form for EACH address, and mail the form(s) to the Board with the fee (check or money order only) to:
