Replace a License
Replace a Lost or Destroyed License
Licensees may only have one original license. A duplicate license may be purchased from the Board to replace a missing one.
Send the following to the Board for a new copy of your license:
- Order form and fee (fees are on the form), and
- A statement establishing the loss or destruction of the license.

New License to Reflect a Name Change
Send the following to the Board for a new copy of your license:
- Order form and fee (fees are on the form), and
- Proof of your name change, e.g., a copy of your marriage certificate or divorce decree.
