Closing a Facility
The owner of a facility may cancel a facility registration if PT and/or OT services will no longer be provided. To cancel a registration, the owner must notify the board in writing (letter, email, fax) that those services are no longer being provided at that location. If the owner decides to resume the provision of those services at a future date, the facility registration may be restored. An owner may not register a new facility in lieu of restoration of a previously registered facility in the same location.
